Forms
Both the Application and Medical Forms must be completed and turned in for participants to be allowed to attend Operation Snowball.Returning participants must fill out a new medical form each year to ensure accurate information.

- PLEASE include Photo Fee with the Participant Registration Fee and submit with the application.
- Fees cover the cost of food, lodging, workshop materials and a souvenir t-shirt.
- Scholarship applications are available upon request.
